The explosion occurred just before 11 p.m., at the Texas Eastern ProductsPipeline Company at Yankee and Todhunter roads, fire officials said.

Authorities said they are 99 percent sure the victim is Kollette Meyer,52, of Waynesville. Firefighters recovered the victim's body.

Fire officials said a spark caused propane leaking from a valve toexplode, but they do not know where the spark came from or why the valvewas leaking.

Fire fighters are standing by as 20,000 gallons of propane burn off.

'We're down to a small tank area, not sure how long it will take to burnoff,' said Monroe Fire Chief Mark Neu. 'It's in a safe area. It's safelyburning. It's being watched.'

There is no danger to residents and no evacuations were ordered, but thesound of explosions and the sight of flames shooting into the skyfrightened people living nearby.

Janis Cravens said the first explosion shook her out of bed. She saidflames shot 50 feet.

'All of a sudden I heard this tremendous boom. As I got up off the bed, Iheard another one. It rolled through my stomach. That's how creepy itwas. I pulled the curtain back and I could see flames,' Cravens said.

'I am shaking inside now. The thought of hearing an explosion like thatand it being so close is just mind-boggling. I look out the window and Isee my worst fear. I go running out of the garage and I see 50 feet offlames,' she said.

Residents from miles around reported a large, orange glow in the nightsky. The terminal is in an industrial area away from homes.

Nearby roads are closed. Yankee Road is closed between Todhunter Road andOxford State Road, and Todhunter Road is closed between Yankee Road andRoute 4.

Fire officials said a valve either broke or was not shut off properly.

Three employees and a supervisor were on the night shift. No one else washurt.

Eastern, based in Houston, is a safety conscious company, firefighterssaid. It has been involved with several training exercises with the firedepartment.


The pipeline through Middletown runs more than 9,000 miles from southernTexas to the Northeast.

Warren County woman killed; cause of blast sought
MONROE — State and local officials are searching for the cause of anexplosion that shook the area surrounding the Texas Eastern loadingfacility at Yankee and Todhunter roads late Sunday, igniting a massivefire that resulted in the death of an employee in training.



While the investigation continued, flames at the Texas Eastern ProductsPipeline Company (TEPPCO) still burned Monday afternoon under thewatchful eye of firefighters.

Kollette F. Meyer, 52, of 8563 Waynesboro Way in Clearcreek Township,Warren County, had been training as an operator at the facility for lessthan two months at the time of the fatal accident. Her job dutiesincluded opening and closing valves and the testing of propane and butaneproducts.

Andy Willis, investigator for the Butler County Coroner’s Office, saidMonday he is “99 percent sure” the worker killed is Meyer. She was one ofthree employees at the plant when the explosion occurred.

She also is the only one who remained unaccounted for Monday afternoon.

“We are waiting for dental records for a positive ID,” Willis said. Thewoman’s skeletal remains were transported to the Butler County morgueMonday morning, Butler County Coroner Dr. Richard Burkhardt said.

Meyer family members could not be reached for comment.

Kathleen Sauve, a spokeswoman for the Houston-based company, declinedcomment without the family’s permission.

“Our employees are, obviously, grieving,” Sauve said. “I just don’t thinkit would be right to approach them at this time.”

Sauve said Meyer was the first TEPPCO employee fatality at the plant,which has been in Monroe since the late 1950s. There was anotherfatality, involving a worker on a contract crew working at the facility,a few years ago, Sauve said.

Julie Martin, a former neighbor of Meyer’s in New Boston, Mich., aDetroit suburb, said she was only just getting to know the Meyers whenthey moved to Ohio. Even still, Martin said it was obvious to her justhow giving a person “Cookie,” as her friends called her, was.

“She was such a generous person,” Martin said. “She helped my daughterget a job and didn’t even know her but recommended her like she was herown. Everyone who knew her knows she was always willing to help others.”

Leonard Mallett, vice president of operations for Texas Eastern, saidthere were several small, residual fires still burning at the facilityMonday afternoon, but that they were under control.

Those fires were expected to burn for most of the day until the propanesupply feeding the flames was exhausted, Mallett said.

Monroe Fire Chief Mark Neu said residents living in the area where theexplosion occurred are “very safe.”

Monroe dispatchers received the first 911 call at 10:54 p.m. from aworker at the TEPPCO site, reporting a fire near the office area. Withinthe next 15 minutes dispatchers and police officers fielded more than 300calls from residents and passersby in the area, Monroe Police Chief GregHomer said.

Neu said flames initially shot into the night sky some 300 feet.

“It looked like daylight. There was a lot of fire,” Neu said.

Monroe dispatcher Danny Rose had just left work and returned to hisBaltimore Street home when the explosion occurred.

“The whole neighborhood shook,” Rose said. “I knew they would be goingcrazy with calls.”


TEPPCO’s TODHUNTER Road storage and loading facility is just one of thecompany’s many delivery points along a massive pipeline that stretchesmore than 11,000 miles from the Texas Gulf Coast to the northeast regionof the United States. There are several dehydration units within thefacility.

TEPPCO owns and operates one of the largest common carrier pipelines ofrefined petroleum products and liquefied petroleum gases in the UnitedStates. It also owns and operates petrochemical and natural gas liquidpipelines and is engaged in crude oil transportation, storage, gatheringand marketing.

Mallett said 7,500 barrels of liquefied petroleum gases pass through theTodhunter facility each day.

TEPPCO officials said company representatives visited Meyer’s familyMonday and sent out notices about the accident to employees company-wide.

Mallett said many of the Todhunter Road facility’s 14 employees were“upset and shaken” by the explosions and fatality. Grief counseling tohelp employees cope will begin today, he said.

“We are doing everything we can to make sure it doesn’t happen again,”Mallett said. “We do have OSHA (the federal Occupational Safety andHealth Administration) on site and we will cooperate with OSHA.”

TEPPCO officials said they would conduct an independent investigation.


NEU SAID it would take a few days before TEPPCO officials had an estimateof the total amount of damage.

He said the preliminary investigation indicates a valve went bad, causingpropane to leak, but that the source of ignition is still unknown.

Neu said the mid-September weather worked in the favor of firefighters.He said the air was “heavy,” holding down the propane.

Neu also praised plant employees for their quick response in shutting offvalves after the explosion occurred.

Fire officials monitored the level of combustible chemicals in the airthroughout the morning hours early Monday, Neu said. As of Monday, noenvironmental or health hazards had resulted from the blast, officialssaid.

The damage to the facility includes scorched buildings and damage totanks.

It will take several weeks for the investigation to be completed and acomplete estimate of damages determined, Sauve said. There was noindication of foul play, she said.

Emergency crews remain at the site around the clock, Sauve said.

“Right now, it’s a controlled burn,” she said. “I don’t know how long thefacility will be closed. We won’t know that until we can get in there anddo an assessment of the site. It’s so early in the time frame that wereally don’t have that many details yet.”

A similar incident occurred at the Monroe facility in May 1993, when 60firefighters from four area departments battled a blaze apparently causedby a leaking pipe. An explosion followed by rolling flames as high as 50feet were reported during that incident. Ten homes along Yankee andTodhunter roads were evacuated.

In May 2003, an 8-inch pipleline owned by Texas Eastern exploded, blowinga hole in Ohio 122 in Clearcreek Township and causing the evacuation of70 homes and the Warren Career Center at Ohio 122 and Ohio 48.

There were no fatalities in either of those incidents.1
